Make your first form
From a blank page to a link you can send, in a few minutes.
A form is a set of questions with a link. You build it in the builder, publish it, and send the link. Answers appear in Results.
Create it
- Sign in and open your Library.
- Press New. A blank draft opens in the builder. The small arrow next to New also offers From a template and Import.
- Click the title at the top and type a name. It reads "Untitled form" until you do.
- Under Questions, press a question type such as Short text or Single choice, then type your question.
- Add more questions the same way. Press More types to see every type.
Choose how it looks
- Open the Design tab.
- Pick a theme. New forms start with Lilac, a lilac page with separate white question cards.
- Pick a Mode: everything on one page, or one question at a time. See Ways to answer.
Publish and share
- Press Preview to try the form yourself. Nothing you enter is saved.
- Press Publish. If something is missing, Chaos lists what to fix first.
- Open the Share tab, press Copy next to Public link, and send it.
- Open Results in the top bar to read answers as they arrive.
